Community Case Management Services at Creative Work Systems are designed to meet the needs of people who live on their own, or with family, and do not receive home supports. Through an individualized, person-centered approach, people are guided in gaining a greater awareness of resources and building deeper community connections. All services are designed to encourage the highest level of home and community based independence.

Everyone receiving case management services is encouraged to have a person centered plan (PCP) written for them. A PCP describes how services and resources will be combined to support goals and needs.

CWS Community Case Managers also offer rep payee services and financial management assistance.
